Title:                   Primary Care Lead 
Salary:                $ 86,778- $102,091 - $114661 per year (pay range explained below)             
Contract:            Term- Until July 2025
Hours:                Standard Hours - 37.5 hours per week
Location:           Chilliwack, Matsqui, or Surrey

About the role
The Lead, Primary Care, is responsible for leading collaborative establishment, coordination and implementation of primary care services, supports and initiatives with Fraser Salish First Nations communities such as the Joint Project Board initiatives and End-to-End Integration programs and services.  The Primary Care Lead helps to enhance the provision of primary care services in collaboration with Fraser Health Authority, Ministry of Health and other partners.

About you
Education
•    A Bachelor's degree in nursing or equivalent clinical background and equivalent combination of education, training and experience.
•    A Master’s Degree in Health Administration, Public Health, Leadership and/or Informatics is an asset. 

Experience
•    Recent, relevant and significant experience (usually acquired over a period of 3-5 years) in a health care environment with a primary care or acute care planning, delivery, project management, strategic planning and community and stakeholder engagement.
•    Experience working with First Nations communities or organizations including political leadership.
•    Experience and knowledge in Regional Health Authority and First Nations Primary Care and virtual health processes, workflows, referral patterns, program delivery models and electronic medical records.

Knowledge
•    Excellent interpersonal and communication skills; ability to foster collaborative partnerships utilizing a team approach
•    High level motivation with a positive attitude and high degree of flexibility in a rapidly changing environment
•    Strong organizational skills and experience in Project leadership/management; ability to organize and prioritize work and balance multiple activities concurrently
•    Strong computer skills and ability to work comfortably with spreadsheets, presentations, diagramming and word processing software (e.g. Microsoft excel, PowerPoint, Visio and Word.
